MGMA Chronic Care Management Service Essentials

As part of an ongoing effort to enhance care coordination for Medicare beneficiaries, the Centers for Medicare & Medicaid Services (CMS) established non-face-to-face chronic care management (CCM) services as a Medicare benefit in 2015.
