Chronic disease remains a major problem within the U.S. healthcare system, and America continues to lag well behind other developed countries in its treatment. But medical practices can make great strides in treating chronic disease by leveraging population health tools. This presentation will explore how practices can implement care coordination into ambulatory care settings to improve outcomes and better control costs in chronic disease patient populations.

A case study of Physicians Medical Center, PC, will illustrate how the practice successfully employed population health tools to manage its diabetes patient population. These tools and capabilities will help other practices implement a winning population health strategy and build a successful chronic care management program. Using proprietary research that examines key pain points and challenges, presenters will share prescriptive insights into how to execute care coordination so that it generates operational, clinical and financial gains.

Claiming Credit:
If you wish to claim continuing education credit, you must first attend the webinar and pass the associated exam with a score of 80% or higher. 

Registration is required for BOTH the webinar and the exam. 
  • ACMPE, medical practice administrators in certification and Fellowship through ACMPE
    • Exam must be completed no later than: March 26, 2021
  • CME, licensed physicians (MD/DO) 
    • Exam must be completed no later than: March 26, 2021 
  • CEU, generic continuing education credit 
    • Exam must be completed no later than: March 26, 2021    

After you register for the exam, instructions on how to access the exam will be emailed to you.  

Participants who want to claim continuing education credit must be individually registered for the webinar and subsequent exam. If registered participants attend as a team, the person who launched the webinar will need to email a list (first and last names) of all participants to after the webinar has concluded. Each participant will be required to complete the exam under his/her own account.   

Team participation is not allowed to claim CME credit for this program.

Continuing education credit is granted to registered attendees who purchase and complete both the webinar and exam.

 American College of Medical Practice Executives (ACMPE)

This program has the prior approval for ACMPE continuing education hours. A cumulative total of 50 ACMPE continuing education credit hours is among the requirements for advancement to Certified Medical Practice Executive (CMPE) status. Once you have advanced to CMPE or Fellow status, you must earn and submit 50 qualifying credit hours every three years to maintain your designation. Thirty hours must come from MGMA national, state or local offerings, twelve being a live learning event.

 Continuing Medical Education (CME)

This activity has been planned and implemented in accordance with the accreditation requirements and policies of the Accreditation Council for Continuing Medical Education (ACCME). The Medical Group Management Association is accredited by the ACCME to provide continuing medical education for physicians. 

The Medical Group Management Association designates this enduring activity for a maximum of 1 AMA PRA Category 1 Credit™. Physicians should claim credit commensurate with the extent of their participation in the activity. 

One CME credit is earned for every 60 minutes of educational content, rounded down to the nearest 0.25.

Continuing Education Units (CEU) 

A certificate of attendance will be provided to healthcare professionals requesting generic CEUs for professional development, certifications through other professional associations or specialty societies.
