MGMA 2012 Patient Centered Medical Home Program

Item #: PCMH12
Location: Chicago, IL
Meeting Dates: 04/23/2012 - 04/24/2012

Visit the MGMA 2012 Patient-Centered Medical Home Program microsite for more program details.

The MGMA 2012 Patient-Centered Medical Home Program is designed for group practice administrators, managers, executives, primary care practice physicians, nurses, nurse practitioners or business accounting professionals involved in managing a patient-centered medical home.

Attendance by administrator and physician teams is encouraged so you can better support each another in undertaking the challenges presented by today’s changing health care systems.

The program is packed with opportunities for you to listen and interact with the experts; collaborate and learn from your peers; prepare your practice for successful transformation; and gain new insights into how the core elements of the PCMH model — care coordination, patient engagement, physician buy-in and certification/accreditation — can be optimized in your practice for immediate results. You will leave with a new appreciation of all the benefits PCMH offers.

All this excitement occurs at the Swissotel Chicago, a magnificent setting just a few blocks off the Magnificent Mile. The Swissotel is located in the heart of downtown Chicago and has breathtaking wrap-around views from Navy Pier to Millennium Park. You will have to see it to believe how incredible it is!

Continue down the path to certification or Fellowship with ACMPE preconference workshops. Attend one or both workshops to help advance your career.

Start making your plans now. See you in the Windy City!
